STM publisher Elsevier, Netherlands, has announced the acquisition of ExitCare, LLC, an enterprise-wide solution for patient education and discharge instructions. ExitCare provides peer-reviewed patient education and discharge instructions to support clinicians, empower patients and help providers meet their patients' educational needs. The acquisition is effective immediately and financial terms of the transaction are not being disclosed.
Focused on improving patient care and outcomes, Elsevier and ExitCare solutions give healthcare providers the tools and information necessary to ensure that patients are discharged in accordance with The Joint Commission and Meaningful Use requirements. ExitCare’s products, incorporated into Elsevier's portfolio of clinical decision support (CDS) content and tools, will help healthcare providers improve the delivery of healthcare information and services by interdisciplinary care teams across all care environments.
Relevant and accurate patient information delivered to patients throughout the care continuum is integral to improving patient outcomes and safety. ExitCare will provide Elsevier with associated print, electronic and video patient education content and tools that are critical to increasing patient engagement and reducing readmissions, and also meet the emerging needs of both hospitals and office-based physicians who wish to provide a multimedia education experience for their patients.
Elsevier and ExitCare first began collaborating in November 2011 on several content solutions, including Mosby’s Nursing Consult, Clinical Pharmacology and MD Consult.
The global publisher's primary clinical constituents use ExitCare’s patient education material to evaluate care, plan for care, facilitate transitions of care, and educate patients on their diseases, medications, labs and all other critical health information available at the point of care. The content is delivered through referential solutions and is also embedded into the clinical workflow through EHR integration.
